Athletes and people in physically demanding jobs that involve long hours on their feet also face elevated risk, simply from the sheer volume of daily stress placed on the tendon.
A prior ankle injury raises your risk considerably as well, even if that injury healed years ago. Scar tissue and altered biomechanics from an old sprain or fracture can quietly weaken the tendon’s support over time, setting the stage for collapse that seems to appear out of nowhere.
Pain and swelling along the inside of the ankle are usually the earliest warning signs, often showing up well before any visible change in arch height. As the condition progresses, you may notice your foot rolling inward more than it used to, a change often visible in worn-down shoe soles. Walking and standing for long periods becomes noticeably harder, and some patients develop pain on the outside of the ankle as the foot’s altered position places new stress there.
Many patients also notice their shoes fit differently, sometimes needing a wider size than before. This shift happens because a flattened arch spreads more of your body weight across a wider portion of the foot.
A collapsed arch changes how force travels through your entire foot and ankle with every step. Over time, this altered mechanics can lead to secondary problems, including bunions, arthritis in the midfoot, and chronic ankle instability. Left untreated long enough, the deformity can become rigid, meaning the foot loses its ability to correct itself even with bracing or orthotics.
